JOB DESCRIPTION
CARE COORDINATOR
Position: Care Coordinator 2
Status: Full-time
Reports to: Administrator on Duty
Qualifications:
High school diploma
Experience in an office setting preferred
Able to pass a State of Arizona Fingerprint/Background Check
Must provide work and personal references
Must be able to lift 30 pounds
Must have driver’s license & proof of car insurance

Responsibilities:
SCHEDULING: Assist in assigning shifts to caregivers; verify all shifts have coverage/review open shifts that need to be filled; verify that shifts have been accepted by caregivers, send reminders as needed; assist Care Coordinator 1 in adjusting schedule as needed; daily review of dashboard; ensure that client & caregiver information is up to date; update client assessments & tasks as needed; resolve any client issues; communicate updates & information to caregivers through T-mail.
CAREGIVER RECRUITMENT/HIRING: Manage job advertisement platforms & job postings; Candidate review, outreach & interview scheduling; Interview, hire, terminate employees in partnership with Care Coordinator 1; oversee onboarding.
CLIENT LIASON: Oversee service inquiries from prospective clients/families, coordinate new client intake; perform regular check-ins with clients on services, update care plans as needed; oversee Electronic Caregiver Fall Detection device activations & delivery; participate in field supervision duties as necessary.
OPERATIONAL DUTIES: Maintain & update the Office Manual; Collaborate with other office staff on operational updates, client/caregiver matters; participate in the paid On-Call & Admin-On-Duty rotation
OTHER DUTIES may be assigned to the Care Coordinator 2 as deemed necessary per the demand of work flow.
Working Conditions
MEAL BREAK PERIODS: An optional minimum of thirty (30) minute/one (1) hour maximum break is available on a daily basis. The time the break is taken must be approved by supervisor and must be clocked in & out for.
Office Setting 5 days per week
Sitting 80% and Standing/Walking/Bending 20%
